Yes! The forms will transfer to ezyVet, however, please be aware that it might not be supported by the older versions of ezyVet, so make sure you're updated to version 21.7 or later to have your forms fully transferred.
You can view which version of ezyVet you are currently using by clicking the Support tab at the right top of your screen:
