First open your Settings App on your iPad:
Scroll down until you find the Smart Flow option, make sure that your Camera toggles are set to On (they are highlighted in green when on):
Then, when in Smart Flow, to take a photo, make sure that you have given access to Smart Flow by tapping Ok:
To record your voice, be sure to Enable Dictation:
